Serivce Agreement

This Service Agreement (“Agreement”) is made between GulfCare Mobile Labs+ we the client who requests mobile phlebotomy services.

By scheduling an appointment or using our services, you agree to the following terms:

1. Services Provided

GulfCare Mobile Labs+ provides mobile specimen collection, including routine blood draws, lab kit collections, swab testing (COVID-19/Flu/Strep), and specimen delivery to partner laboratories. We do not provide medical diagnosis, treatment, or medical advice.

2. Client Responsibilities

You agree to:

- Provide accurate personal and health information

- Maintain a clean, safe, and private environment for collection

- Follow pre-collection instructions such as fasting or hydration

- Remain still and follow directions during the procedure

- Provide valid payment information

3. Payment Terms

- Payment is due at the time of booking unless otherwise stated.

- Fees cover collection only, not laboratory processing charges.

- Laboratory fees, insurance billing, or provider billing are not handled by GulfCare Mobile Labs+.

- No-show fees may apply if you are not available at your scheduled time.

4. Specimen Handling

We ensure proper handling, labeling, and delivery of your specimen. However, we are not responsible for:

- Lab processing errors

- Lost specimens once delivered to the laboratory

- Delays caused by laboratory operations

5. Results

Results are provided by the processing laboratory or your ordering provider. We can provide results to you only if permitted by the ordering lab or provider.

6. HIPAA & Confidentiality

All personal and health information is protected under HIPAA. We do not disclose your information without your consent unless required by law.

7. Limitation of Liability

GulfCare Mobile Labs+ is not liable for:

- Reactions or side effects from blood draws

- Lab errors, delays, or lost specimens

- Misinterpretation of laboratory results

- Medical decisions made using the results

Your use of our services is at your own risk.

8. Cancellations & Refunds

- Cancellations must follow the policy stated during booking.

- Late cancellations or no-shows may result in a fee.

- Completed services are non-refundable.

9. Governing Law

This Agreement is governed by the laws of the State of Florida.

10. Acceptance

By scheduling an appointment, you acknowledge that you have read and agreed to this Service Agreement.
